Eyelid surgery or blepharoplasty is the surgical removal of skin and sometimes fat on the upper and lower eyelids to address droopy eyelids and eye bags. Eyelid surgery has received highly satisfactory results from both surgeons and doctors.

The surgery is carried out as an outpatient procedure under local anaesthesia with sedation. You will be able to return home on the same day of the surgery.

Preparations before the surgery are necessary as with all cosmetic surgery. Firstly, a consultation with the surgeon is required to determine the specific goal and need of the patient. After the consultation, a pre-anaesthesia consultation will be conducted. Blood tests are also necessary to rule out any medical problem that can impede the flow of the surgery and the recovery period. Once everything is in order, the patient can now proceed in scheduling for the surgery.

On the day of the operation, fasting is a must from midnight. The length of the surgery typically lasts around one to one and a half hour.

After the surgery, tiny dressings will be placed on the surgical wound to aid in closure. Minimal bruises and swelling are expected for the first couple of weeks. Also, intermittent ice pack application is recommended for the first day to reduce swelling and bleeding. Oozing of blood is not uncommon for the first day. However, it is best to return immediately for a follow-up with the surgeon should any prolonged bleeding persist.

The stitches are removed after four or five days and a small dressing is applied. This dressing can then be removed after a day.

Tummy tuck or abdominoplasty is an effective treatment for addressing excess fat, loose skin and tighten the abdomen. THE DIFFERENCE

MINI ABDOMINOPLASTY

Tightening of the muscles are also performed through permanent stitches along the central part of the abdomen. This will eventually give a flatter and tighter tummy.
Since the full abdominoplasty addresses both upper and lower part of the abdomen, the procedure is twice as long as the mini abdominoplasty.
Two drains are attached on the lower abdomen to help remove excess fluid and prevent fluid accumulation (seroma).

COMPLICATIONS AND RISKS

Minor complications are swelling, bruises and fluid accumulation (seroma). Swelling and bruises usually disappear after a few weeks. Seroma can be treated through aspiration of the fluid through a needle attached to as syringe. There is also the risk of scar and keloid formation.

Although very rare, major complications can still occur. The most critical is the death of the tissue. This can be prevented by careful planning and taking a detailed medical history of the patient. Smoking and intake of certain medications are few factors that can affect and slow down the progress.

Penis Enlargement and Lengthening (Penoplasty)

The phallus has been a symbol of masculinity since time immemorial. This may be the reason that men seek to increase the size and shape of the penis in order to somehow prove masculinity and of course to enhance self-esteem.

Penoplasty is an aesthetic procedure that involves lengthening and enlargement of the penis. The procedure usually takes around 2.5 to 3 hours and is done under local anaesthesia with sedation. Sedation is a type of deep sleep anaesthesia wherein patients are not aware of the surgery but will still be able to respond with stimuli and the breathing is not compromised. Unlike general anaesthesia wherein total patient response is diminished and spontaneous breathing is assisted through intubation or other assistive breathing devices.

In penoplasty, penis lengthening is achieved through an incision on the pubis to release a ligament that acts as a mobile point for the penis during an erection. This release of the ligament increases the size of the penis to 1-3 cm during flaccid. However, it is important for the patient to understand that the length will be the same during the flaccid and erected state.

Meanwhile, penis enlargement is done through the injection of one’s own fat. Initially, the surgeon will collect fat from an area where you have abundant of them, like abdomen or flanks, by liposuction. The collected fat tissues are then filtered and removed of excess fluids. For additional viability of these fat tissues, stem cells or platelet-rich plasma may be added. Finally, the surgeon will inject the collected fat tissues into the penis using a small cannulae.

The usually expected side complications will be mild to moderate pain and discomfort, bruising and swelling of the penis and the donor area. Age, gravity, fluctuations in weight and decrease in elasticity can contribute to the lowering of the breasts over time. The amount of drooping will depend on the volume and elasticity of the breast tissues.

Breast uplift surgery is an effective way to reshape the breasts making them look lifted, firmer and more youthful in appearance. This type of surgery is for patients who are experiencing moderate to severe ptosis (droopy) of the breasts and who are not planning to have more children. Although there are no risks associated with pregnancy or breastfeeding after breast uplift surgery, there is the possibility of the skin stretching again and will render the surgery ineffective.

THE SURGERY, ANAESTHESIA METHOD, AND RECOVERY

The surgery is usually done under local anaesthesia with moderate sedation, but can also be under general anaesthesia. Moderate sedation is a drug-induced depression of consciousness wherein patients are able to respond to verbal commands either alone or accompanied by movement stimulation. Spontaneous breathing is observed in sedation unlike in general anaesthesia where the breathing ability is compromised thereby requiring airway assistance devices like intubation. The surgery will last approximately 3 to 3.5 hours and recovery in the clinic will take around 2 to 4 hours and can be longer depending on the amount of time for the patient to recover.

AFTER BREAST UPLIFT

After the surgery, you will have an adhesive bandage dressing around the breasts which will act as a support as well as maintain the shape of the breasts. This adhesive bandage dressing will be removed in 4 days. A cotton, non-wired bra is worn for a period of 1 month. The stitches will be removed in a couple of weeks. The breasts will look pointy and high for the first few weeks but will eventually settle down and will look more natural.

IMPORTANT CARE INSTRUCTIONS

Refrain from stretching the arms above shoulder length so as not to stretch the incisions. Sleeping on the back is required for a month. Strenuous exercise should be avoided until one month.

Breast augmentation with implants is an effective way to increase the size of one’s breasts. It is considered as one of the most sought aesthetic procedures all over the world.

How will I know which breast implant size is suitable?

The breast implant size will be based on the outcome you want to achieve. Recently, more women are deciding to have natural-looking breasts.

During the consultation, a physical examination will be conducted as well as a breast simulation using a one-of-a-kind technology which can help you visualise the different sizes of implants.

Where are the implants placed?

The implants are either placed between the breast tissues and the muscles (subglandular) or below the muscles (submuscular).

Where will the incisions be?

The incisions will be placed on the natural crease of the skin, which is the fold under the breasts. The incisions will be around 4cm long. The incisions will flatten in a few months and will be less conspicuous.

How is the surgery done?

Breast augmentation is done under local anaesthesia with sedation. During sedation, you will be asleep and will not be able to remember the operation. The whole procedure will take around 1.5 to 2 hours.

What happens after the surgery?

You will be expected to remain 2-4 hours after the surgery. This may be longer depending on how fast you recover.

There will be a dressing as well as an adhesive bandage around the breasts which will be left in place for 4-5 days.

Once this adhesive tape is removed, a regular non-wired cotton bra is recommended and will be worn for a month.

When can I go back to my daily activities?

As with all surgeries, resting for the first couple of days is advised. It is for your own safety to have someone assist you for the first 2 days. Driving is also not allowed during these 2 days because of the sedative medications.

You can start exercise on the lower body 2 weeks after the surgery and upper body for 4 weeks.

It is important that in order not to stretch the incision scars, you are advised not to raise your arms above shoulder level for a month.

Adipose-derived Stem Cells (ADSC): A short overview of its benefits and uses

What is adipose-derived stem cells?

Stem cells use has been a complex subject since the late 1990s. However, in 2001, researchers (Zuk and colleagues) at the University of California, Los Angeles, identified and described the multipotent stem cells from liposuctioned fat tissue. They first named these stem cells as Processed Lipoaspirate Cells or LPA Cells. Since then, the term LPA Cells has evolved to Adipose-derived stem cells (ADSC).

The potential benefits of ADSC are promising in the regenerative medicine because of several advantages:

Ability to continue rapid reproduction after transplantation
To release growth factors
To induce neovascularization (natural formation of new blood vessels)

How is it done?

The first step is the collection of the fat (around 50-100 mL, depending on the requirement of the laboratory). This can be collected in the body area that have been agreed by you with the surgeon. This collected fat will be processed under sterile conditions in the laboratory. It will take a few days to a couple of weeks for the stem cells to be ready. The quality of cell integration will depend on the intrinsic characteristics of the patient. Once ready for use, the laboratory will contact the clinic in order to proceed with the main treatment.

What is ADSC useful for?
